It's not about Heroin ...the song started as a mish-mash of lyrics based on newspaper clippings Lennon had read i.e the story about the pervert with the mirrors attached to his hobnail boots looking up girls skirts...the "i need a fix cos im goin down" and "happiness is a warm gun" lyrics were just throwaway lines that john used....as he often did.The "i need a fix" lyric is about his drug taking but the song as a whole isn't just to clear that up.His only song about Heroin is Cold Turkey.

Yeh this is all about heroin, and can be percieved in two ways, that it is either happieness or that it is a darker substitute.
The song may also have some sexual undetones as with 'mother superior jumped the gun' etc.
